Blood sugar, or glucose, is the amount of sugar in your blood. It comes from the food you eat. Your body needs it for energy, but too much can cause problems. Uncontrolled blood sugar levels can lead to type 2 diabetes or worsen your disease.

Carrots can be a safe choice if you have diabetes and are watching your blood sugar levels. They’re also non-starchy vegetables. So you can even enjoy small amounts of carrots if you’re following the ketogenic, or keto, diet.

Glycemic Index

This measures how much some foods and drinks raise your blood sugar levels. It runs on a scale of 1 to 100. A score of 100 means the food has the same effect on your body as eating a type of sugar called glucose.

The lower the glycemic index (GI), the slower your blood sugar rises. Raw carrots have a GI of 16. The GI for boiled carrots ranges from 32 to 49. That puts carrots in the low glycemic food group:

  • Low glycemic index: 1-55
  • Medium glycemic index: 56-69
  • High glycemic index: 70 or higher

The glycemic index for any food will go up if you cook or prepare them with honey or other carbohydrates. Still, carrots are high in fiber, so that helps slow down how quickly they release the sugar. They also have a lower glycemic index than other root vegetables like potatoes.

Glycemic Load

The glycemic index isn’t the only number you should watch. Another is glycemic load. It combines the glycemic index with the serving size to give you a total picture of the effect on your blood sugar. Eating low glycemic index food but a lot of it will raise the glycemic load.

Two small raw carrots have a glycemic load of about 8. That also puts carrots in the low glycemic load group:

  • Low glycemic load: 1-10
  • Medium glycemic load: 11-19
  • High glycemic load: 20 or higher

Type 2 Diabetes Remission: New Guidelines Outline the Steps to Follow With a Doctor

It may not be easy to reach the point where type 2 diabetes is so well controlled that it’s considered in remission, and it’s not likely in every case, but it’s possible for some people. New guidelines from a group of U.S. and international medical organizations, published in August 2021 in Diabetes Care, lay out the steps that are necessary to get there.

Many strategies patients use to manage type 2 diabetes — including lifestyle changes, medication, and weight-loss surgery — can help them get to remission, according to the guidelines, which were drafted by members from the Endocrine Society, the European Association for the Study of Diabetes, Diabetes UK, and the American Diabetes Association. Whatever intervention, or combination of approaches, people use, the guidelines note, there’s still one target they need to hit for doctors to consider the disease in remission: an A1C of below 6.5 at least three months after stopping medication under a doctor’s supervision. A1C is a three-month average of blood sugar levels. (The recommendation does not apply to people with type 1 diabetes, who must take insulin for life to manage blood sugar.)

“If they (people with type 2 diabetes) artificially lower blood glucose with drugs, you cannot see if the disease process has been halted,” says Michael Lean, MD, a diabetes researcher and nutrition professor at the University of Glasgow in Scotland, who also involved in the guidelines.

Anita Courcoulas, MD, MPH, chief of minimally invasive bariatric and general surgery at the University of Pittsburgh Medical Center in Pennsylvania, who wasn’t involved in drafting the guidelines, adds that it takes about three months for this process to be verified when a patient is taken off medication.

When Doctors May Consider Remission Potential for a Person With Type 2 Diabetes

The choice of whether to take a patient off medication to verify remission isn’t always straightforward for doctors. Ultimately, it depends on the patient, says Caroline Messer, MD, an endocrinologist at Fifth Avenue Endocrinology in New York City.

“I look at the trend for my patients,” says Dr. Messer, who wasn’t involved in the guidelines either. “If they’re on medication and their numbers go below 6.5 and are consistently dropping, and they’re also making a concerted effort with lifestyle changes, then I would stop medication for three months and then test again to see if A1C rises back up or not.

Why Remission From Diabetes Doesn’t Mean Abandoning Healthy Habits

If A1C does climb above 6.5 percent again, a person would need to go back on medication, Messer says. If it doesn’t, Messer likes to tell them that they now have diet- and exercise-controlled diabetes.

“If you gain weight, if you stop exercising, if you change your diet, diabetes can return,” Messer says. In other words, even if a person has entered what’s considered remission from diabetes, they can’t completely let go of diabetes-friendly diet and lifestyle habits.

The chances of remission can vary with the approach to managing diabetes, and remission isn’t always permanent.

For example, a review published in Nutrients in April 2019 found that weight loss surgery may lead to remission in 80 percent of patients at one year, but only 45 percent of them sustained remission after five years.

Similarly, a study published in July 2019 in The Lancet Diabetes & Endocrinology found that intensive weight management with low-calorie meal replacements could lead to remission in 46 percent of participants after one year, but remission rates slipped to 36 percent after two years.

When Is Type 2 Diabetes Considered in Remission?

How soon doctors can consider diagnosing remission can vary based on the methods used to get blood sugar levels on target, the guidelines stress.

For example, a person who lowers their blood sugar to a healthy range after undergoing metabolic surgery — a weight loss procedure done in part to help individuals with overweight or obesity manage conditions like diabetes — must be at least three months post-surgery before they can be considered in remission.

People who don’t use surgery or medication to manage diabetes — and instead make lifestyle changes like overhauling their eating and exercise habits — must stick with these new routines for at least six months before they’re considered in remission.

“The longer time period of six months for the effect of lifestyle therapy is due to the fact that weight change is slower after lifestyle interventions compared to metabolic surgical interventions, so this time period allows for the full effect of the lifestyle intervention to be realized,” says Dr. Courcoulas.

Remission is more likely when people receive treatment to lower their blood sugar soon after diabetes gets diagnosed, says Matthew Riddle, MD, lead author of the guidelines and a professor of endocrinology, diabetes, and clinical nutrition at Oregon Health and Science University in Portland. Therefore, remission may be easier to reach with an earlier diagnosis.

People who try multiple approaches at once — like medication and lifestyle changes — may also have a better shot at remission, Dr. Riddle says.

Screenings After Type 2 Diabetes Remission

Once individuals are in remission, in addition to following healthy habits, they still need to have their A1C levels tested at least once a year, the guidelines advise, and no more than every three months.

This is in part because people may not always maintain any weight loss that helped them achieve remission, the guidelines note. Testing is also necessary because researchers aren’t yet certain how long remission can last or what factors may contribute to a relapse.

Beyond checking blood sugar, people with diabetes need ongoing monitoring by a diabetes specialist.

In the period shortly after remission, individuals have an increased risk of diabetes complications due to the rapid drop in blood sugar that helped them achieve remission, the guidelines note. Down the line, there’s also a risk that complications could develop, especially when blood sugar rises to levels slightly above normal, to an A1C above 5.7 percent but less than 6.5 percent, which signals prediabetes.

After Remission, Diabetes-Related Complications May Linger

It’s not yet clear whether people in remission can avoid vascular complications when their blood sugar levels are in the prediabetes range, Courcoulas says. Vascular complications can include damage to the blood vessels in the eye that impacts vision, diabetic nerve damage that can lead to loss of sensation or impaired motor control in the limbs, and reduced kidney function.

People in remission may fare better over the long term when they keep up the habits that got them there, Courcoulas says.

“Exercise regularly, maintain a healthy lifestyle, and see your treating doctor for ongoing monitoring of both A1C levels and for possible relapse or complications of diabetes, even if you have experienced remission,” Courcoulas advises.

Diabetes Drug Tied to Slower Cognitive Decline in Alzheimer’s

A class of type 2 diabetes drugs is associated with reduced brain plaque and better cognitive scores in people with Alzheimer’s disease, a new study suggests. The findings appear in the August 2021 issue of Neurology.

The drugs — called dipeptidyl peptidase 4, or DPP-4, inhibitors — are prescribed to people with type 2 diabetes to help lower blood sugar. Examples of DPP-4 inhibitors that are approved by the Food and Drug Administration are Januvia (sitagliptin), Onglyza (saxagliptin), Tradjenta (linagliptin), and Nesina (alogliptin).

“People with diabetes have been shown to have a higher risk of Alzheimer’s disease, possibly due to high blood sugar levels, which have been linked to the buildup of amyloid beta in the brain,” said senior study author Philhyu Lee, MD, PhD, of Yonsei University College of Medicine in Seoul, South Korea, in a statement.

“Not only did our study show that people taking dipeptidyl peptidase-4 inhibitors to lower blood sugar levels had less amyloid in their brains overall, it also showed lower levels in areas of the brain involved in Alzheimer’s disease,” Dr. Lee said in the statement.

At the start of the study, researchers gave 282 participants with Alzheimer’s disease brain scans to measure the accumulation of amyloid plaque, or abnormal protein deposits in the brain that are associated with memory decline. They also administered cognitive tests and repeated assessments roughly every 12 months over a median follow-up period of 2.5 years. The study included three cohorts: 70 people with diabetes treated with DPP-4 inhibitors; 71 people with diabetes who didn’t take DPP-4 inhibitors; and 141 people without diabetes.

Brain scans showed that people on DPP-4 inhibitors had a lower amyloid burden than either of the other two groups in the study after researchers adjusted for age, sex, education, cognitive status, and a mutation of the APOE-4 gene associated with Alzheimer’s disease.

Individuals with diabetes also experienced smaller declines in global cognition and memory recall when they took DDP-4 inhibitors than when they didn’t use these drugs.

People on DPP-4 Inhibitors Had Better Cognitive Scores

Activities on the cognitive test repeated during the study included tasks like counting backwards from 100 by sevens or copying a picture. Scores on the test, called the Mini-Mental State Examination (MMSE), ranged from 0 to 30, with higher scores indicating better cognitive function. The average annual decline in scores was 0.87 points for people with diabetes on DPP-4 inhibitors, 1. 65 points for people with diabetes not on these drugs, and 1.48 points for people without diabetes.

“I’d suspect that by lowering amyloid there might be some effect on reducing memory decline,” says Glen Finney, MD, the director of the memory and cognition program and a neurology professor at the Geisinger Commonwealth School of Medicine in Scranton, Pennsylvania.

“A bigger study would help to see if that were the case,” adds Dr. Finney, who wasn’t involved in the study.

Beyond its small size, the study was limited because researchers lacked data on baseline blood sugar levels, which would be needed to help determine what role the changes in blood sugar over time might play in the results.

The study also didn’t use brain scans to look for changes in amyloid accumulation over time. In addition, the MMSE tests administered multiple times, while considered an accurate way to track changes in cognitive function, don’t offer the same precision as more comprehensive cognitive assessments such as neuropsychological testing, Lee and colleagues note in their study.

Earlier Studies Link Diabetes Drugs to Slower Cognitive Decline

Yet previous studies have also linked diabetes medications to better outcomes for people with Alzheimer’s disease, says Finney.

“Over the decades there’ve been hints in research that at least some Alzheimer’s disease cases relate to glucose and diabetes, and may be helped by treatments that are used for diabetes,” Finney says. “This is another example of a treatment, DPP-4 inhibitors, that is showing some signal of that.”

One study published in August 2020 in Alzheimer’s & Dementia, for example, looked at memory test results for 1,192 people with type 2 diabetes who didn’t have cognitive impairment and for 807 people with type 2 diabetes who had Alzheimer’s disease. This study found that DPP-4 inhibitors were linked with a slower deterioration in memory among those with Alzheimer’s disease. It also found that Glucophage (metformin), another type of medicine to lower blood sugar, was associated with slower memory decline among carriers of an APOE-4 gene mutation linked to Alzheimer’s disease.

An earlier study, published in June 2018 in the Annals of Translational Medicine, reviewed preclinical lab work and human studies to date on the effect of DPP-4 inhibitors on cognition and found that DPP-4 inhibitors were protective against further memory decline in people with diabetes who had mild cognitive impairment.

DPP-4 inhibitors are used to improve the regulation of blood sugar, which preclinical studies suggest may be associated with increased amyloid accumulation in the brain, says Jill Morris, PhD, assistant professor of neurology at the University of Kansas Medical Center in Kansas City, who wasn’t involved in the latest study.

“Bringing blood sugar levels back to normal could potentially normalize amyloid processing,” Dr. Morris says.

More Research on Diabetes Drugs and Alzheimer’s Disease Are Needed

Randomized clinical trials are needed to answer this question and to determine if DPP-4 inhibitors can directly cause slower cognitive decline, Morris adds. That’s because DPP-4 inhibitors are often prescribed for people whose blood sugar is not ideally managed and who don’t respond to other diabetes medicines. As a result, people on DPP-4 inhibitors may have higher blood sugar levels when they initiate treatment than patients who don’t use these drugs — this was the case for the patients in the latest study.

“Randomized clinical trials are needed to evaluate the effectiveness of DPP-4 inhibitors to slow cognitive decline in individuals both with and without diabetes,” Morris says.

While all people with type 2 diabetes should still take medications as needed to lower their blood sugar, including DPP-4 inhibitors if they’re prescribed, people shouldn’t be on this medication just because they think it might help prevent dementia, Finney says.

“It is too early to make any decisions on taking DPP-4 inhibitors solely for hope of slowing Alzheimer’s disease, but if you have a good reason otherwise to be on a DPP-4 inhibitor, then we can hope you may get an added benefit,” Finney says. “But whether or not to use a DPP-4 inhibitor should be based on what is best for your diabetes control.”

Managing blood sugars as a diabetic is a lot more complicated than just avoiding sweets. Many seemingly-healthy foods can cause a rapid rise in blood glucose, and carrots are often listed among that number. In truth carrots are a healthy option for diabetics despite their naturally sweet flavor, and they can be eaten regularly as part of your balanced diet.

The GI Misperception

Many diabetics and health conscious non-diabetics use a tool called the Glycemic Index, or GI, to help manage their blood sugar levels. The GI compares how much a fixed quantity of a given food raises blood sugars, comparing it against the same quantity of pure glucose. As the reference point, pure glucose is assigned a GI of 100. The higher the GI of a specific food, the more it boosts your blood sugar, so the lower the number the better.

Carrots currently are assessed at a GI of 41 by the Glycemic Index Institute, which is a moderate level, but earlier and less-stringent testing resulted in a higher figure. This made intuitive sense, given that carrots taste naturally sweet, so people with diabetes have often been warned to limit their consumption of carrots because of that flawed early test.

They’re Part of a Healthy Diet

There’s more to a food than its impact on your blood sugars though, and by any reasonable standard carrots are a superbly healthy mealtime option. A half-cup of cooked carrots supplies over two grams of dietary fiber – nine percent of your daily value – in its six grams of carbs, with just 27 calories. You’ll also get more than double your daily value of vitamin A, and 13 percent of your day’s vitamin K.

That one-cup portion of raw carrot provides three grams of fiber, or 14 percent of your daily value, against 12 grams of total carbs and 50 calories. You’ll also get more than four times your daily value of vitamin A, 13 percent of your vitamin C, 21 percent of your vitamin K, and 12 percent of your daily value of potassium. Both raw and cooked carrots pack smaller quantities of most other key vitamins and minerals your body needs.

They’re Versatile

Foods for specialized diets sometimes represent a lot of additional cost and preparation time, but carrots definitely don’t fall into that category. They’re inexpensive, readily available all year round and versatile enough to fit in with almost any style of cooking. They can be shaved into green salads or shredded for slaw, incorporated raw into a stir-fry or cooked in a frittata or enjoyed on their own as a carrot soup or as an ingredient in other soups and stews. Enjoy them steamed or boiled as a side for traditional meals, or even roasted as an accompaniment to your traditional Sunday dinner or holiday turkey.
