What Causes Bed-Wetting in Adults, and How Can You Treat It?
A lot of folks think bed-wetting is something that only happens to kids, but it’s a problem that can hit grown-ups, too. You may feel embarrassed to wake up to wet sheets, but it’s not your fault. It could be due to a medical condition, medicine, or a problem with your bladder. You’ve got lots of ways to fix it.
Causes
If you start wetting the bed as an adult, see your doctor. You may hear them call your problem nocturnal enuresis, which is the medical name of the condition. Some of the reasons it may be happening to you:
Your kidneys make more pee than normal. A hormone called ADH tells your kidneys to make less urine, and you normally make less of this hormone at night. When you have bed-wetting issues, you may not make enough of this hormone or your kidneys might not respond well to it.
A form of diabetes called diabetes insipidus also affects ADH levels, causing you to make more urine.
Your bladder can’t hold enough urine. When there isn’t enough room in your bladder, pee can leak.
Overactive bladder (OAB). Your bladder muscles normally squeeze when you’re ready to pee. In OAB, these muscles squeeze too often or at the wrong times.
Medicine. Some drugs you take can irritate your bladder, such as sleeping pills or antipsychotics like:
Your bed-wetting may also be due to conditions that affect your body’s ability to store and hold urine. For instance, bladder cancer and prostate cancer can cause it. So can diseases of the brain and spine, such as a seizure disorder, multiple sclerosis, or Parkinson’s disease.

How Is Bed-Wetting Diagnosed?
Your doctor will do an exam and ask about your symptoms and health history. Keep a diary so you’ll have the answers to their questions. Write down things like:
- How often and what time your bed-wetting happens
- How much urine comes out (a lot or a little)
- What and how much you drank before bed
- Any other symptoms you’ve had
Your doctor will do tests to diagnose the problem, such as:
Urinalysis. It checks a sample of your urine to look for an infection or other conditions of the urinary tract — the collection of organs that are involved with urine like the kidneys, ureters, bladder, and urethra.
Urine culture. Your doctor sends a small sample of your urine to a lab, where technicians put it in a special dish with nutrients. This test looks for bacteria or yeast in your urine. It can diagnose a urinary tract infection.
Uroflowmetry. You pee into a special funnel to measure how much urine you make and how quickly it flows out.
Post-void residual urine measurement. This test measures how much urine is left in your bladder after you pee.
How Do You Treat Bed-Wetting?
Your doctor may suggest you start by making a few changes to your daily and nightly routines:
Try bladder retraining. Go to the bathroom at set times during the day and night. Slowly increase the amount of time between bathroom visits — for example, by 15 minutes at a time. This will train your bladder to hold more fluid.
Don’t drink right before bed. That way, you won’t make as much urine. Avoid caffeine and alcohol, which can stimulate your bladder.
Use an alarm clock. Set it to wake you up at regular times during the night so you can use the bathroom.
Try a bed-wetting alarm system. You attach it to your underwear or a pad on your bed. It will alert you as soon as you start to wet the bed.
Take medicines. Several can help with bed-wetting. Desmopressin (DDAVP) reduces the amount of urine your kidneys make.

If medicines and other treatments don’t work, your doctor might recommend one of these procedures:
Bladder augmentation. It’s an operation that makes your bladder larger, which raises the amount of urine it can hold.
Sacral nerve stimulation. It helps control an overactive bladder. Your doctor puts a small device into your body that sends signals to nerves in your lower back that help control the flow of urine.
Detrusor myectomy. It’s a major operation that treats an overactive bladder. Your surgeon removes part or all of the muscles around your bladder to stop them from contracting at the wrong times.
Tips to Deal With Wetness
Until you can get bed-wetting under control, take some simple steps to manage the situation:
- Put a waterproof cover or pad over your mattress or sheets to keep them dry.
- Wear absorbent underwear or pads to bed.
- Use special skin cleansing cloths and lotions to prevent your skin from getting irritated.
If you try one treatment and it doesn’t work, go back to your doctor. Sometimes it takes a few tries to find the right solution to bed-wetting.
Purpose, Procedure, Types, and Recovery
A woman might feel an ache in their lower abdomen. Or they may have trouble peeing. Maybe sexual intercourse is uncomfortable. If you experience any of these, the problem might be your bladder.
The bladder sits in sort of a “hammock” of muscle and tissue inside your abdomen. When the tissue gets weak, the bladder can break through it and go into your vagina. Doctors call this bladder prolapse. It’s a common condition, and it’s treatable.
Symptoms
If you have a mild case, you may not even realize it. But if you do have symptoms, here’s what they might include:
- You feel pressure in your pelvis or vagina
- When you cough or lift something, you feel discomfort in your abdomen
- You have repeated bladder infections
- Your urine may leak out unexpectedly. Or, when you pee, the stream might be weak or take a long time.
- You might feel pain during sex
- You have trouble putting in tampons or applicators
- If you have an extreme case, tissue may bulge through the opening of your vagina. It might make you feel like you’re sitting on an egg.
The symptoms might be strongest if you have to stand for a while. When you lie down, they may ease.
When you discuss bladder prolapse with your doctor, they may call it by another name: cystocele.
Your doctor might also discuss a condition that affects the tube that carries urine from your bladder outside your body. It’s called the urethra, and it also can break through its support and into the vagina. When that happens, it’s called urethrocele. It is not unusual for it and cystocele to occur at the same time.
Should You Have Surgery?
If your symptoms are making your life difficult, surgery may be the answer. You need to think about a few things:
Your age. If you’re young, keep in mind that bladder prolapse can happen again. Even if you have surgery now, you may end up needing another operation down the road. If you’re older, any other health issues you have may affect your decision.
Your plans about having children. If you want more children, it’s best to hold off on surgery until you decide your family is complete.
Your general health. Surgery has some risks, such as infection, blood clots, or problems with the anesthetic. If you have a heart condition, diabetes, or breathing difficulties, any of these things could make an operation riskier. So could smoking or obesity.
Also, talk with your doctor about the chances that surgery could cause new problems. You might develop urinary incontinence — urine leaking out without your control. Sex might become uncomfortable, and you might feel pain in your pelvis.
Types of Surgery
There are two types of surgery for bladder prolapse. The surgeon might:
Close off or narrow the vagina to create support for the bladder. This makes sexual intercourse impossible. Doctors call this obliterative surgery.
Build back the tissue that holds up the bladder. To do this, the surgeon might use the body’s own tissue or insert artificial mesh. Doctors call this reconstructive surgery.
Let’s look at reconstructive surgery more closely:
The surgeon has a few options. They might rebuild the support for the bladder by using your body’s own tissue. Or, they might insert a mesh and attach it to a bone. A mesh has risks though, including the chance that it may cause an infection or deteriorate over time. So doctors generally restrict it to special situations — if a woman had previous surgery that failed, or if their own tissue is too weak to rebuild.
Depending on what the surgeon plans to do, they may work through a cut in your abdomen. That might involve a technique called laparoscopic surgery. For that, they makes very small cuts and uses special instruments through the openings. Or, instead of cutting into your abdomen, the surgeon might operate through the vagina.
If you have surgery, the medical team may put you to sleep. Or they may use a drug that makes only the affected part of your body numb.
After the Surgery
The time it takes to recover depends on what the surgeon does. For the first few weeks, you should avoid strenuous exercise and sexual intercourse. You’ll probably have to take a few weeks off from work.
Treatment without Surgery
As you think about surgery, you also should consider available therapies and treatments that don’t involve an operation.
Exercises might strengthen the muscles that help hold the bladder in place. They’re called Kegel exercises. To do them, you squeeze and release the muscles that cut off the flow of urine.
A device called a pessary might help support your bladder. This is a plastic or rubber ring that your doctor fits especially to you. It’s a common alternative to surgery, especially if surgery would be risky for you. Your doctor would show you how to clean it and put it back in place.
Estrogen therapy might help if you’ve already gone through menopause. Estrogen helps the muscles in the pelvis stay strong, but the body produces less of it after menopause. Replacing it with an estrogen cream, pill, or ring would make up for that loss.
A mild case of bladder prolapse may not need treatment at all. Your doctor might suggest that you just wait and see if the symptoms get worse.
Bed-wetting – Symptoms and causes
Overview
Bed-wetting — also called nighttime incontinence or nocturnal enuresis — is involuntary urination while asleep after the age at which staying dry at night can be reasonably expected.
Soggy sheets and pajamas — and an embarrassed child — are a familiar scene in many homes. But don’t despair. Bed-wetting isn’t a sign of toilet training gone bad. It’s often just a normal part of a child’s development.
Generally, bed-wetting before age 7 isn’t a concern. At this age, your child may still be developing nighttime bladder control.
If bed-wetting continues, treat the problem with patience and understanding. Lifestyle changes, bladder training, moisture alarms and sometimes medication may help reduce bed-wetting.
Symptoms
Most kids are fully toilet trained by age 5, but there’s really no target date for developing complete bladder control. Between the ages of 5 and 7, bed-wetting remains a problem for some children. After 7 years of age, a small number of children still wet the bed.
When to see a doctor
Most children outgrow bed-wetting on their own — but some need a little help. In other cases, bed-wetting may be a sign of an underlying condition that needs medical attention.
Consult your child’s doctor if:
- Your child still wets the bed after age 7
- Your child starts to wet the bed after a few months of being dry at night
- Bed-wetting is accompanied by painful urination, unusual thirst, pink or red urine, hard stools, or snoring
Causes
No one knows for sure what causes bed-wetting, but various factors may play a role:
- A small bladder. Your child’s bladder may not be developed enough to hold urine produced during the night.
- Inability to recognize a full bladder. If the nerves that control the bladder are slow to mature, a full bladder may not wake your child — especially if your child is a deep sleeper.
- A hormone imbalance. During childhood, some kids don’t produce enough anti-diuretic hormone (ADH) to slow nighttime urine production.
- Urinary tract infection. This infection can make it difficult for your child to control urination. Signs and symptoms may include bed-wetting, daytime accidents, frequent urination, red or pink urine, and pain during urination.
- Sleep apnea. Sometimes bed-wetting is a sign of obstructive sleep apnea, a condition in which the child’s breathing is interrupted during sleep — often due to inflamed or enlarged tonsils or adenoids. Other signs and symptoms may include snoring and daytime drowsiness.
- Diabetes. For a child who’s usually dry at night, bed-wetting may be the first sign of diabetes. Other signs and symptoms may include passing large amounts of urine at once, increased thirst, fatigue and weight loss in spite of a good appetite.
- Chronic constipation. The same muscles are used to control urine and stool elimination. When constipation is long term, these muscles can become dysfunctional and contribute to bed-wetting at night.
- A structural problem in the urinary tract or nervous system. Rarely, bed-wetting is related to a defect in the child’s neurological system or urinary system.
Risk factors
Bed-wetting can affect anyone, but it’s twice as common in boys as in girls. Several factors have been associated with an increased risk of bed-wetting, including:
- Stress and anxiety. Stressful events — such as becoming a big brother or sister, starting a new school, or sleeping away from home — may trigger bed-wetting.
- Family history. If one or both of a child’s parents wet the bed as children, their child has a significant chance of wetting the bed, too.
- Attention-deficit/hyperactivity disorder (ADHD). Bed-wetting is more common in children who have ADHD.
Complications
Although frustrating, bed-wetting without a physical cause doesn’t pose any health risks. However, bed-wetting can create some issues for your child, including:
- Guilt and embarrassment, which can lead to low self-esteem
- Loss of opportunities for social activities, such as sleepovers and camp
- Rashes on the child’s bottom and genital area — especially if your child sleeps in wet underwear

Bed wetting causes many practical problems, such as constantly having to change wet sheets and bedding. Bed wetting can affect staying away overnight, going on holiday or business trips. Bed wetting can affect sleep patterns and often it causes frustration and exhaustion.
Bedwetting can also affect self-esteem which can put a strain on personal relationships.
Types Of Nocturnal Enuresis
Persistent Primary Nocturnal Enuresis is a condition which starts during childhood, where night time dryness has not been achieved for longer than six months. About 2 – 3% of adults over 18 years of age have this type of nocturnal enuresis. Adult Onset Secondary Enuresis is defined as nocturnal enuresis in which night time dryness has been achieved at some point in life. Dryness may have occurred for many years but then night time wetting suddenly begins at an older age.
What Causes Bedwetting (Nocturnal Enuresis)?
There are many causes of bedwetting and others remain unclear. Usually people produce less urine when they are sleeping. However, some people produce larger amounts of urine during the night, which may explain why the bladder needs emptying then.
So why don’t you wake up when you need to go to the toilet? Unfortunately, there is no clear cut answer to this question.
Research has shown that nocturnal enuresis can occur due to a genetic link. Though this is not true for everyone, evidence has shown that bedwetting is hereditary. One study showed that someone with two bedwetting parents has a 77% chance of also becoming a bedwetter. When one parent wet the bed as a child, then their child was found to have a 40% chance of becoming a bedwetter.
Another cause put forward for primary nocturnal enuresis is a smaller than average bladder. This does not mean that the actual size of the bladder is smaller in nocturnal enuresis patients, rather that their functional bladder capacity (FBC) is a smaller volume. The FBC is the amount of urine the bladder holds before sending a signal to the brain to indicate the need to void. The overactivity of the detrusor muscle (bladder) indicates that the muscle is never fully relaxed during the filling phase and therefore the bladder capacity is not as large.
Some people have an ‘overactive’ or ‘unstable’ bladder (OAB) which causes problems at night as well as during the day. Several studies have found a high incidence of detrusor instability (OAB) with nocturnal enuresis. Detrusor overactivity has been found in up to 70 – 80% of primary nocturnal enuresis patients.
The amount and type of drinks that you have is important. We all need fluids to stay healthy, but some drinks can irritate the bladder or make the body produce urine more quickly than usual. Alcohol, and drinks that contain caffeine (such as tea, coffee, hot chocolate and cola), can affect you in these ways.
Some medicines prescribed for completely unrelated problems can change the way your urinary system works and can cause you to have less control than normal. These medications include common drugs used for heart and blood pressure problems, or for mental illness and anxiety.
An infection in the urine (urinary tract infection, ‘UTI’) can sometimes cause bed wetting. Stress or anxiety can also cause the problem, which might last long after the stress has gone.
If you start bed wetting again as an adult and this persists, it could be the result of a more serious underlying problem. If this is the case, you need to go to the doctor immediately for further investigation.
What Help Is Available?

Waking up in urine-soaked sheets is not anyone’s idea of a good time, especially if you’re sharing the bed. Urologist James Ulchaker, MD, helps explain what’s going on in your body that’s causing this involuntary bed-wetting after too many drinks.
1. Alcohol suppresses a hormone in the brain
Nocturnal enuresis, or nighttime bedwetting, can happen when you overindulge because alcohol affects several things in your body that make it more difficult to hold your pee.
We are all equipped with an antidiuretic hormone (ADH) produced by the brain. ADH signals the kidneys to keep them from making too much urine, thereby preventing you from using all of your hydration reserves. Drinking alcohol suppresses ADH production, so your body produces more urine than it normally would.
“When you’re awake, you make up for extra urine by making more trips to the bathroom,” Dr. Ulchaker says. “If you pass out later or are sleeping too soundly, your bladder continues to fill more quickly and gets over-distended. Your body then releases the pressure by urinating,” he says.
But if you’re drinking to excess or late into the night, this suppression of ADH can continue long after you hit the hay. The result is your body is producing a whole lot more urine while you’re dozing than it normally does when you’re sober … which can result in wet sheets.
2. Alcohol irritates a key muscle in your bladder
Alcohol also irritates the detrusor muscle, part of the wall of the bladder that signals when you need to pee. If you’re passed out with inhibitions lowered from a drunken state, you may miss the signals from this muscle and void in your slumber.
3. Extra fluids build up faster
The volume you’re consuming might play a role in increasing the amount in your bladder. Sharing a pitcher of beer during a game or dinner with friends adds significantly to the normal amount of liquids you take in.
But add to this the simple fact of when we usually drink alcohol: evening time! This means you’re drinking closer to the normal time you go to bed, and this can dramatically increase the amount of pee your bladder has to hold through the night. This can really be a problem as you turn out the lights, especially if you already have a difficult time holding your pee under normal circumstances.
What’s more, Dr. Ulchaker says drinking alcohol causes your body to pull from its liquid reserves, which can lead to dehydration and add even more volume in your bladder.
But a main factor with your bladder’s involuntary contractions is caffeine, Dr. Ulchaker says.
4. Caffeine activates the bladder
Remember that muscle in the bladder? The detrusor? Caffeine makes that muscle contract, telling you to pee before the bladder is even full. This allows you to hold less and less urine over time. Because caffeinated beverages so often go hand-in-hand with alcohol (think rum and Coke), this is a double-whammy for your bladder.
An extra precaution to prevent accidents
Of course, drinking too much alcohol isn’t healthy, especially if it’s affecting your ability to hold your urine on a regular basis. If you have an occasional extra drink or two, there are a few ways to reduce your risk of wetting the bed while you sleep.
- Avoid caffeine before a night out. If you know a night of drinking is in store, avoid extra caffeinated beverages throughout the day or limit the amount of liquids.
- Switch from large quantities to smaller ones. Instead of that pitcher of beer, consider a gin and tonic or other mixed drink.
- Make an extra trip to the bathroom right before bed. Be sure to void your bladder completely right before hitting the pillow.
- Set a middle-of-the-night alarm. You may find that limiting fluids and avoiding caffeine doesn’t help. If you still aren’t able to reliably hold your urine until morning after a night out, Dr. Ulchaker recommends setting a wake-up call during the night to control bed-wetting. This may be annoying, but if it prevents you from waking up in a wet bed, it may be worth it.

Why Do I Leak Urine When I Sleep?
