Q&A: Preconception vs. Prenatal Vitamins

As part of a blog series on fertility nutrition, we interviewed SGF’s Medical Director, Dr. Eric Widra, to discuss the difference between preconception vitamins and traditional prenatal vitamins.

We have known for years that a woman’s nutrient needs prior to conception are different than they are throughout pregnancy. Dr. Widra recommends that all SGF patients take an appropriate preconception vitamin while trying to conceive.

What is the main difference between preconception vitamins and prenatal vitamins?

According to Dr. Widra, “Preconception vitamins are designed to set the stage for a healthy pregnancy. As women transition into pregnancy their requirements are different.”

It is important to start taking a preconception vitamin as soon as you begin trying to conceive.

Taking a high-quality preconception vitamin, along with eating a healthful diet, helps prepare your body for conception and a healthy pregnancy.

What nutrients do you think are most important to prepare any woman’s body for pregnancy?

A preconception vitamin should provide a complete range of nutrients including folate (folic acid), choline, iodine, and vitamin D. Dr. Widra explains that the primary requirements for a preconception vitamin include having adequate folic acid and vitamin D, and overall good nutritional support.

Folic acid is a B vitamin that is involved in many of the body’s normal processes. Reducing the risk for certain birth defects is perhaps folic acid’s most well-known benefit. These birth defects are called neural tube defects.

The neural tube, which eventually forms into the brain and spinal cord, closes at approximately 28 days after conception. A neural tube defect results when the tube does not close properly. Because the neural tube closes during the early weeks of pregnancy, taking folic acid while you are trying to conceive is very important. Research shows that taking 400 mcg of folic acid for at least 1 month before pregnancy can reduce the risk of neural tube defects by about 36 percent.

When a person consumes folic acid, the body converts it to the metabolically active form called methylated folate. Although Dr. Widra points out that most patients have normal folate metabolism, some women have a genetic defect (called MTHFR polymorphism) that reduces their ability to activate folic acid. A preconception supplement with methylated folate may be of benefit for these women, particularly while trying to conceive and through early pregnancy.

Why is vitamin D so critical for preconception?

Believe it or not, vitamin D is still a somewhat controversial topic. However, studies have generally demonstrated better in vitro fertilization (IVF) success rates in women with normal vitamin D levels (>30 ng/ml) compared with those with low levels. One study found a 6 percent increase in clinical pregnancy rate with each ng/ml increase in vitamin D level.

Dr. Widra explains, “With those data in hand, and the very low cost and low risk associated with using vitamin D, Shady Grove Fertility tests patients’ vitamin D level and recommends supplementation if it is low.

Do you recommend any specific supplements for women with advanced maternal age or diminished ovarian reserve?

There are limitations to the data supporting the routine recommendation of any specific supplements for these patients. However, for patients who really want to do something, or have found their way to the supplement world on their own, we do offer them coenzyme Q10 or DHEAS.

Coenzyme Q10 (CoQ10)

Many women look to CoQ10 because it plays a crucial role in energy production inside cells (including egg cells). CoQ10 levels decline with age. Therefore, a woman’s eggs become less efficient at producing energy as she ages. This is thought to contribute to poorer egg quality, which can adversely affect fertility and pregnancy outcome.

This Fertility and Sterility study explains that nutrients such as CoQ10 can increase cellular energy production, which may have a positive impact on fertility in older women trying to conceive. Animal data also supports CoQ10’s role in promoting egg quality as women age.

Enrolling participants in fertility studies can be difficult, and there has only been one human study examining CoQ10 supplementation for female fertility. Although the results were promising, the study was too small for the findings to be statistically significant.

Dr. Widra further explains that if a woman wants to give CoQ10 a try, he likes Theralogix because of their superior formulation and favorable price point.

DHEAS

Dehydroepiandrosterone Sulfate (DHEAS) is a hormone produced in the adrenal glands. DHEA production declines with age. Although the use of DHEAS for female fertility is somewhat controversial, research has shown that it may improve IVF success in women with diminished ovarian reserve.

When appropriate, women can take 75 mg of DHEAS for at least 4 weeks, and up to 4 months prior to IVF.

Do you see a benefit of using inositols for your patients with PCOS?

Yes. The studies on inositols and PCOS are relatively convincing that they help promote ovulation and an ovulatory response to medications. Therefore, SGF recommends inositols for its patients with PCOS.

In addition, studies have found that inositols reduce insulin resistance and decrease testosterone levels in women with PCOS. Inositols may also promote egg quality and ovarian response to medications in women undergoing IVF.

Research shows that taking a combination of two forms of inositol (myo-inositol and D-chiro-inositol) in the body’s naturally occurring ratio of 40:1, is more beneficial than taking either form alone.

When do you recommend that your patients switch to a traditional prenatal?

A woman’s nutritional needs change as she transitions from preconception and early pregnancy to the second trimester. Therefore, all SGF patients are advised to switch to a traditional prenatal vitamin by 10 weeks of pregnancy.

What do you tell your patients to look for in a traditional prenatal that they might not need in their preconception vitamins?

Dr. Widra explains that there is good evidence that supplementing with DHA after the first trimester has benefits for pregnancy in terms of both fetal development and reduction in preterm labor and delivery. Also, a woman’s iron requirements increase during the second and third trimesters of pregnancy.

Docosahexaenoic acid (DHA)

DHA is an omega-3 fat found in certain fish and algae. DHA supplementation during pregnancy has been associated with improved birth weights and a reduced risk for preterm delivery. DHA may also support fetal mental and visual development. A traditional prenatal should meet the recommended daily intake of 250-300 mg a day.

Iron

A woman’s iron needs are generally the same while trying to conceive and during the first trimester of pregnancy as they are for any premenopausal woman. A preconception supplement should contain 18 mg, the Recommended Dietary Allowance (RDA) for premenopausal women.

Iron needs increase during the second and third trimesters of pregnancy due to greater iron use by the developing baby. A traditional prenatal should contain 27 mg of iron, the RDA for pregnant women.

Prenatal Vitamins Vs Preconception Supplements

For decades, physicians have recommended women of childbearing age take a daily prenatal vitamin with folic acid to lower the risk of birth defects of the brain and spinal cord, called neural tube defects. Once conception occurs, most women then continue with the same prenatal vitamin through the duration of their pregnancy. While this approach to prenatal vitamins is not new, recent studies have shown that a woman’s nutrient needs prior to conception are different than during pregnancy. Further research has shown the impact of particular vitamin levels on the chances of success with fertility treatment. Certain supplementation may increase the chances of success.

Not All Vitamins Are Created Equal: Preconception Vs. Prenatal Vitamins

Effectively preparing a woman’s body for treatment may optimize her chances of conception. Physicians at Shady Grove Fertility recommend that women start taking a preconception supplement while trying to conceive and then transition to a traditional prenatal vitamin around 10 weeks of gestation.

Vitamin D

Having a normal vitamin D blood level is critically important for conception. Studies have shown that women with normal vitamin D levels are up to four times more likely to achieve a pregnancy while undergoing in vitro fertilization (IVF) compared with women who are vitamin D deficient. 1-4Most women undergoing fertility treatment should be taking at least 2,000 IUs of vitamin D3daily.5

Iron

Iron requirements increase substantially during the second and third trimesters of pregnancy, but while you are trying to conceive, taking extra iron may actually hurt your chances of conception since it can lead to higher levels of oxidative stress in the ovary. Most prenatal vitamins have more iron than is needed prior to conception. In addition, the high level of iron in many prenatal vitamins can cause stomach upset and constipation.

DHA

Many prenatal vitamins contain DHA, an omega-3 fatty acid. DHA supplementation provides several benefits once you are pregnant but is not needed during fertility treatment and may be an unnecessary added cost prior to conception.

Coenzyme Q10

Coenzyme Q10 (CoQ10)plays a key role in cellular energy production. Our bodies make CoQ10 in virtually every cell, but CoQ10 production declines with age. Emerging research suggests that supplementing with CoQ10 may enhance energy production in the ovary. 6-7

DHEA

A DHEA supplement may be recommended for women who produce too few eggs during IVF treatment, or who have concerns about egg quality. DHEA is a hormone produced in the adrenal glands and when used under a physician’s guidance, is safe and potentially effective. A small number of research studies have shown improved pregnancy rates in women using DHEA during treatment. 8

Inositols

Inositols are sugar molecules that are already part of our diet. Research has shown that supplementation with two types of inositols—d-chiro inositol (DCI) and myo-inositol (MI)—can help restore regular periods and ovulation and normalize hormone levels in women with polycystic ovary syndrome (PCOS).  9-11 While inositols are not part of the standard preconception supplementation they can be taken in conjunction with a preconception vitamin to help support the hormone balance in women with PCOS.

How Do I Pick A Prenatal?

Why is a Prenatal So Important?

Prior to conception, the more we nourish our bodies, including taking quality prenatals, the more we decrease risk for birth defects and increase our chance for a successful pregnancy and healthy baby. Some women prefer to wait until they are pregnant to focus on their nutritional intake, but RMA of CT supports the approach that women and their partners should optimize their health many months prior to pregnancy. Men and women have higher fertility success and healthier pregnancy outcomes when they invest in their own health.   Practicing a healthy lifestyle, even 8-12 weeks prior to conception, can also make a significant difference in the health of mother and baby. So, go for it, assuming you’re trying to conceive or even thinking about it, choose that prenatal now!

Where Do We Even Begin?

When a woman is faced with the task of choosing a prenatal, this decision can feel like choosing a cereal in a grocery aisle of 100 boxes. The U.S FDA categorizes vitamins as “dietary supplements,” so the regulation of vitamins is not as strict as what prescription medications go through in the country. For example, the supplement industry has no standardized serving sizes or nutrient dosing to ensure safety.

Simply, the FDA does not have the resources for inspection and supervision of the supplement world. Consumers should be aware that manufacturers don’t even need approval from the FDA to sell their supplements before they are marketed unless it contains a new product ingredient that has not been marketed before 1994. All that might sound a little unnerving, but the industry can’t go completely rogue on us!

Manufacturers are expected to follow the 2007 Good Manufacturing Practices (cGMP) to ensure supplement identity, purity, quality, strength, and composition.  It is also up to the manufacturer to notify the FDA if there have been any consumer complaints or side effects with any supplement.

All this makes it that much tougher to choose a prenatal. 

I do believe that preconception & prenatal care should be a shared responsibility between healthcare providers and their patients. I strongly recommend that all supplements (vitamins, herbs, and other supplements) be approved by your reproductive endocrinologist and/or obstetrician/gynecologist, or nutritionist/dietitian prior to consumption.  Do not rely on internet reviews.

When Should I Take a Prenatal?

Prenatal vitamins are very beneficial taken daily prior to fertility treatment, pregnancy, and preferably 3 months (or more) before conception. If you’re actively trying to conceive, start the prenatal regimen now.

And guess what? This goes for men, too! Folic acid/folate, which you’ve probably heard of at this point, is found in a prenatal, but also is an important nutrient for healthy sperm.  All men who are actively trying to conceive will benefit from a basic men’s multi-vitamin containing 100% of the RDA or a specific men’s conception-based formula.

Which Prenatal is Best?

I would like to share a few steps that I take when helping my patients choose prenatals based on quality, safety, cost, and personalized dosing.

How to Choose a Prenatal Vitamin:

1. Observe Daily Serving Size

The number of daily pills or capsules are prescribed on the bottle. Serving sizes can range from one to even up to six pills daily.  This information is very important because taking less than the minimum recommendation may increase risk for birth defects or maternal nutrient deficiency such as anemia.  Also, confirm that the daily number of pills listed is realistic for you.

Also, more supplementation than recommended on the bottle is not better or safe to you or your baby.

2. Avoid a Gummy Option

Gummy vitamins may be easier to swallow for some, but usually with a gummy multi-vitamin or prenatal, many essential nutrients are either missing or they are provided at a lower than recommended dose. Iron is not at all found in a gummy vitamin. In addition, key prenatal nutrients like DHA, iodine, zinc and others are also not adequate in a gummy.

3. Avoid prenatal vitamins with added herbs

Many herbs are contraindicated in pregnancy.

4. Consider your personal preferences and/or medical needs, ie. cultural, religious, or medical. Options are available.

  • Does the prenatal need to be vegan/vegetarian, kosher, allergen-free?
  • Do you require special dosing or additional supplements? For example: Gluten free/celiac, allergies, bariatric surgery, vegan/vegetarian, anemia, MTHFR, seizure disorder, eating disorder, nutrient deficiency, spina bifida

5. Compare Cost

Keep in mind that some bottles will only cover one month or many times 2-6 months. Sometimes what appears to be a higher cost is actually the best deal!

What Are the Key Nutrients I Should Look for in a Prenatal?

Let’s talk about folic acid/folate a little more…

Folic acid, folate or methylfolate–found in a prenatal–are all a different form of the same essential B vitamin, that plays a key role in preventing birth defects of the brain and spine, such as spina bifida. The neural tube develops into the baby’s brain and spinal cord within the first 28 days of conception. 

A birth defect can occur if the neural tube does not close and this is called a neural tube defect.  This critical time period of fetal development is so early in the pregnancy that most women and couples do not know that they are pregnant—another reason to be taking a quality prenatal many weeks or months prior to conception.  Folic acid/folate is also necessary for DNA synthesis of red blood cells, nervous system, and proteins; and cellular growth and reproduction.

What is the difference between FOLIC ACID, FOLATE, and METHYLFOLATE?

Folic acid is the synthetic form of vitamin B-9. This synthetic version is found in multi-vitamins, prenatal vitamins, and used to fortify foods such as breakfast cereals, pasta, and breads. Once consumed, folic acid is converted by the body into folate. Since folic acid has been added to grain products in the US–together with a greater emphasis on taking prenatals with folic acid—there has been a 50-70% reduction of neural tube defects. 

Folate is a naturally occurring form of vitamin B-9 found in whole foods. Excellent sources of folate include foods such as leafy, dark green vegetables, citrus fruits, lentils and beans, asparagus, peanuts and wheat germ.

Methylfolate is a more bioavailable or highly absorbable form of vitamin B-9This option is very beneficial for anyone who has difficulty absorbing folic acid or needs a clinically high dose for a medical reason.

Could taking too much folic acid be a problem?

Yes. Do not take more folic acid than recommended by your physician or nutritionist. A mega dose of folic acid, without a medical reason, can mask a B12 deficiency, causing pernicious anemia and irreversible nerve damage. Methylfolate, a bioactive form, will not mask B12 deficiency.

Should I try to eat foods naturally rich in folate?

Yes. Folate is found naturally in foods. Excellent sources of folate include foods such as leafy, dark green vegetables, citrus fruits, lentils and beans, asparagus, peanuts and wheat germ.   Incorporating these foods into a daily meal plan, in addition to taking supplemental folic acid from a prenatal vitamin, is essential and beneficial for conception, healthy pregnancy, and healthy baby.

Why not just eat food sources rich in folate instead of taking supplement folic acid?

The naturally occurring folate in food sources is important too, but it is not wise to rely on food-based folate intake alone for the following reasons:

  • Food based “folate” has a forty percent lower absorption rate compared to folic acid or methylfolate. If folate, not folic acid or methylfolate, is listed on your supplement bottle, then it is advised to switch to a supplement that lists folic acid or methylfolate instead of folate.
  • Folate in foods is easily destroyed when foods are cooked or canned. Fifty percent of food folate is lost in the canning process and up to ninety percent can be destroyed in cooking.  Lightly steaming instead of boiling in water is recommended. 

Why should I still take a supplement with folic acid even if I think I’m getting a good amount of folate from food?

  • Grains are the main food group that receives folic acid fortification. The lower carbohydrate meal plans restrict grains, thus lowering folic acid intake.
  • Be advised that many gluten free grains are not enriched with folic acid and other B vitamins and iron.
  • Ethnic foods found in the US are often not fortified with folic acid. This is especially true for corn masa flour—which is more popular among Hispanics—and is not fortified.  In fact, Hispanic women are twenty percent more likely to have a child with a neural tube birth defect than a non-Hispanic white woman.
  • Organic food companies are not required to fortify grain products with folic acid. Some do, but many do not. Check the labels of your favorite organic grain products. 
  • Folic acid /folate is a water-soluble nutrient, which means that our bodies do not store this B vitamin. Availability is dependent upon an individual’s daily nutrient intake.
  • Some medications and regular intake of alcohol can interfere with folate metabolism and other medications can reduce the serum and tissue concentration of folate.
  • Some vitamins/supplements can interfere with medication absorption and interaction. On the flip side, some medications can also interfere with supplement absorption. Always disclose and discuss your vitamin, supplement and medication list and your risk for interactions with your prescribing physician and your pharmacist.  Some specific examples are…
     
    1. If you’re taking thyroid medicine in the morning, it is best to take all vitamins and supplements in the evening as not to interfere with the thyroid medicine.
    2. Calcium supplements should be 4 or more hours away from thyroid medicine, prenatal and iron supplements.
    3. Herbal supplements may interfere with medication and RMA of CT does not recommend herbal supplements during fertility treatment. RMA of CT also does not recommend taking any essential oils or herbal supplements during the 1st trimester of pregnancy.
  • If a woman has already had a child with a neural tube defect, her risk for having another is very high. She should speak with her RMA of CT physician or ob/gyn to get a high dose prescription folic acid.

“Prenatal vitamins are important before and during pregnancy to help your body meet the demands of pregnancy and aid in your baby’s development,” says Dr. Nwankwo. “Your balanced diet should be the foundation of your nutritional vitamins. Your body is able to absorb more from your diet than from supplements. However, your prenatal will help fill in the gaps.”

A run-of-the-mill multivitamin won’t cut it when you’re pregnant. Prenatal vitamins are specially designed multivitamins that deliver the extra amounts of nutrients needed during various stages of your baby’s development. For example, they contain folic acid, which is important for baby’s brain development in the earliest stages. And many have vitamin D and calcium to build strong bones and teeth during the third trimester of pregnancy.

Plus, prenatal vitamins help make sure you have enough nutrients such as iron, to not only provide to your baby but so your own body doesn’t become depleted.

When Should I Start Taking Prenatal Vitamins?

You should start taking prenatal vitamins at least a month and ideally three months before conception and throughout your entire pregnancy from four weeks pregnant to whenever labor and birth starts.

In fact, it’s often best to take a prenatal vitamin every day if there’s a chance you could get pregnant at all, even if you’re not planning for it. That’s because crucial neurological development takes place during the first month of pregnancy, when folic acid would offer the most protective benefits. “I recommend starting prenatal vitamins as soon as you start thinking of trying to conceive. Ideally three months before conception,” Dr. Nwankwo says.

If you’re planning on breastfeeding, Dr. Nwankwo recommends you continue to take your prenatal vitamins for a minimum of about six months, if not throughout.

If you’re already pregnant and haven’t been taking prenatal vitamins, no need to worry! Just start as soon as you can.

How Do I Choose Prenatal Vitamins?

Prenatal vitamins are readily available over the counter and a prescription is generally unnecessary. (Note: check with your insurance to see if they cover prenatal vitamins. If so, you’ll likely need your doctor to give you a prescription for them.)

If you’re concerned about which to choose, don’t be. “There’s not much difference between over the counter and prescription prenatals, and both are adequate,” assures Dr. Nwankwo.

“Sometimes prescription prenatals may have special formulations—for example, they may have a stool softener added to help with constipation, or they may have additional iron. The pills may also be smaller and easier to take. But it’s not necessary,” she says. “It also doesn’t matter whether you take a tablet or a gummy, just make sure it has the right amount of vitamins you need.”

Every vitamin can differ in the types and amounts of nutrients it contains because specific vitamin ingredients aren’t regulated by the FDA. If the vitamin you choose doesn’t contain the full recommended daily amount of a nutrient, be sure to eat foods that are high in that vitamin or mineral.

When shopping for prenatal vitamins look for these four primary nutrients that are especially important during pregnancy, per Dr. Nwankwo and the American College of Obstetricians and Gynecologists):

  • Folic acid: 600 mcg (Helps with neural tube development and can help prevent neural tube defects like Spina Bifida.)
  • Iron: 27 mg (Helps prevent anemia, which can affect delivery of oxygen to the baby via red blood cells. Keep in mind that most gummies don’t contain iron.)
  • Calcium: 1,000 mg (Helps with bone health. Note that most prenatal vitamins don’t contain this much calcium, so if you don’t get enough from your diet, talk to your doctor about taking a separate calcium supplement.)
  • Vitamin D: 600 IU (Helps support healthy bone development and your immune system.)

What Type of Prenatal Vitamins is Best?

Vitamins come in multiple forms: pill, soft-gel capsule and gummies. For some, swallowing and holding down a pill can be difficult especially when you’re queasy due to morning sickness. If that’s your situation, try a soft-gel capsule or a gummy vitamin. Just remember that gummy vitamins do not contain iron, so you may want an extra iron supplement for that.

  • Science and doctors back vitamins — like folate and vitamin D — that provide the body with nutrients that help support a healthy pregnancy.* These may be covered by insurance, depending on your provider.
  • A prenatal vitamin with folate should be taken a month before trying to conceive (at a minimum!) to support fetal neural tube (brain and spine) development — but depending on the formulation, some prenatals may be balanced enough that you can take them way earlier than that. *
  • What science and doctors don’t support: supplements that claim to treat medical conditions like anovulation and infertility. For conditions like these, it’s better to stick to evidence-backed, FDA-approved treatments.

What’s considered a vitamin or supplement?

The words “vitamin” and “supplement” are often used interchangeably, but they’re not quite the same thing. Here’s the low down: Vitamins are naturally occurring nutrients, and while we should aim to get vitamins from the foods we eat, they can also be taken in pill/tablet/liquid/powder form. (Think vitamin A, vitamin C, etc.)

Over-the-counter vitamins have a single vitamin, and multivitamins have — you guessed it — multiple vitamins. Supplements are products that often have a combo of vitamins, minerals, and phytonutrients in them. (So… vitamins can be included in supplements — but not all supplements are vitamins. )

How are vitamins and supplements regulated?

In the United States, the Food and Drug Administration’s (FDA) job is to monitor our food, medications, medical devices, and cosmetics to make sure these products are safe for use and consumption. This monitoring process looks really different across categories — for example, the FDA’s monitoring process for mascara is entirely different than its monitoring process for your prescription birth control.

The FDA’s rules and regulations for vitamins and supplements are the same. According to the FDA, something falls under the category of “vitamin” or “supplement” if it is not marketed for the purpose of treating, curing, diagnosing, or preventing a disease or medical condition.

This is where the Federal Trade Commission (FTC) steps in: The FTC looks at the truth and accuracy of any claims made in dietary supplement advertising and marketing — and the FTC and FDA work together to monitor claims made on websites or in other online marketing. Vitamins and supplements can’t be marketed with claims like “will treat infertility” because that would be seen as a treatment/cure. Despite these regulations, it’s always important to be a critical and careful consumer when evaluating your options for vitamins and supplements.

Here, we’ll review two main classes of products marketed toward women planning to conceive: prenatal vitamins and supplements, and infertility supplements.

Prenatal vitamins and supplements

Prenatal vitamins and supplements get two thumbs up from the medical community. Even for those of us eating the healthiest of meals, our food likely falls short of several crucial vitamins that are important for developing follicles and embryos.

Your typical prenatal supplement contains good stuff like folate, vitamin D, calcium, and iron, and it can be beneficial to take one even if you aren’t actively trying to conceive (TTC). It’s also possible (though not certain) that if you take a women’s daily supplement not specifically formulated for prenatal health, adequate amounts of this good stuff may already be included.

When should you start prenatals?

You should begin taking prenatal supplements before actively trying to have kids (or undergoing IVF procedures) because it may take time for the vitamins and minerals to get to optimal levels in the body. The American College of Obstetricians and Gynecologists’ (ACOG) official rec is to get in 600 micrograms (mcg) of folate per day during pregnancy. But because folate can be hard to get enough of from food alone, ACOG suggests starting a prenatal with at least 400 mcg of folate at least one month before pregnancy to support fetal neural tube (brain and spine) development.*

Though there’s variability between brands, many prenatals (including our Prenatal Multi) have the same all-star lineup of active ingredients. Let’s briefly get acquainted with each.

Ingredient 1: Folate

Prenatal folic acid (a form of folate) supplementation is a big deal — such a big deal that it’s been praised as one of the greatest public health breakthroughs of the 20th century. A 1991 study of folic acid supplementation among women trying to conceive was actually stopped midway through because folic acid was so effective at supporting fetal neurodevelopment that they decided to give it to all of the women in the study.*

The recommended minimum amount of folate you should look for in a prenatal is 400 mcg so you can make sure you’re getting that full 600 mcg a day (when combining supplementation with folate-rich foods). There are some reasons why more than the typically recommended amount of folate might be suggested to you (like medical history), so if you have questions about what’s right for you, make sure to talk to your doctor.

Folic acid, folate, or methylfolate?
You may be hearing some buzz about the differences between folate, methylfolate, and folic acid in prenatal supplements.

Folate, also known as vitamin B-9, is the umbrella term for all forms of the nutrient, including the lab-made versions used in supplements:

  • Folic acid is the form of folate commonly found in processed foods, prenatal vitamins, and other dietary supplements.
  • Methylfolate (aka L-methylfolate, 5-MTHF, 5-methyltetrahydrofolate) is a biologically active form of folate, meaning it’s easy for your body to absorb.

Ingredient 2: Iron

Iron is the building block for hemoglobin, which is a protein in blood cells that shuttles oxygen to organs and tissues throughout the body. The amount of blood you have increases by 30% to 50% during pregnancy; translated into raw numbers, if you had about 4,500 milliliters of blood while not pregnant, you’d have between 5,850 and 6,750 milliliters (mL) when pregnant. Extra iron is required to support this extra blood volume — recommended iron levels rise from 18 mg for nonpregnant adults to 27 mg during pregnancy — but national health data suggests that many people enter pregnancy already at an iron deficit (also called anemia).

To avoid iron deficiency during pregnancy, ACOG recommends all pregnant people get 27 mg of iron a day. * You can either get your daily iron levels from prenatal vitamins, iron-rich foods, or a combination of the two.

Ingredient 3: Choline

Prenatal nutrition has focused almost exclusively on folate (aka folic acid) for supporting fetal neural tube development.* However, recent research suggests that choline is equally essential in neurodevelopment, as well as for many other crucial functions.*

Juxtaposed to the increasing awareness of the importance of choline, national health data shows that 90%-95% of pregnant people in the US are not getting enough. How much is enough? For adults with ovaries, the recommended amount of choline is 425 mg per day. If you’re pregnant, ACOG’s recommendation increases that level to 450 mg per day. During lactation (whether or not you’re breastfeeding/chestfeeding), needs shoot up even higher to 550 mg per day. This is because growing another human is nutrient intensive — as that brain grows and develops in utero and postpartum, prenatal choline is required to build the brain cells and neural pathways.*

Widespread choline deficiency is partially attributable to the lagging supplement industry, as few prenatal vitamins provide choline — but the Modern Fertility Prenatal Vitamin is one of the few that does include it (along with 11 other key nutrients for before, during, and after pregnancy). We packed in as much choline as possible, 100 mg, while keeping the daily dose to 2 capsules and maintaining a balanced formulation of other nutrients recommended by ACOG.

Ingredient 4: Vitamin D and/or calcium

Vitamin D and calcium work together to promote the development of bones and teeth, and further work together in a second way: Vitamin D actually helps with calcium absorption. * Because developing babies get their calcium and vitamin D from their birthing parents while pregnant, it’s important for everyone’s health to have adequate levels of these compounds. What are adequate levels? 600 international units (IU) of vitamin D, and/or 1,000 milligrams (mg) of calcium.

Many prenatal multivitamins don’t have calcium in them — including the Modern Fertility Prenatal Multivitamin. That’s because calcium can block the absorption of iron and absorption of calcium supplementation maxes out at ~500 mg. Both of these factors can make it difficult to put calcium alongside other nutrients in a prenatal. Instead, Modern Fertility opted for the calcium booster vitamin D.* (Read more about the decision here.)

Comparing prescription and over-the-counter prenatal vitamins

No matter how you get your prenatal vitamins, chances are they’ll have similar main ingredients. The main differences between prescription versus over-the-counter (OTC) prenatals come down to small differences in doses and how you pay for them.

Prescription prenatals often contain closer to the upper end of the recommended amount of a vitamin or mineral. For example, prescription prenatals may contain closer to 1,000 mcg of folic acid compared to the 400 mcg found in many OTC formulations — though the Modern Fertility Prenatal Multivitamin contains the full 600 mcg of folate recommended during pregnancy.

Depending on your insurance plan, prescription prenatal vitamins and supplements may be covered. You can also use your FSA and HSA dollars (refresher on what those are here) toward OTC prenatal vitamins. (Fun fact: You can also use those dollars for all Modern Fertility products, including the Prenatal Multi!)

It’s always a good idea to talk to your doctor (especially if you have any diagnosed medical conditions) before making any decisions or starting a supplement regimen to figure out what’s best for you.

Infertility supplements

Here’s where things get a little less… scientific and thus, recommendations aren’t so clear. The supplements that we talked about above are supplements that help support a healthy pregnancy, but they don’t make any claims about helping you get pregnant in the first place. There are, however, a whole host of supplements that do make such claims — claims like “promotes natural fertility,” “improves ovulatory performance,” and “improves your pregnancy chances.” Now, don’t get us wrong: It would be exceedingly great if there was scientific evidence to support supplements doing those sorts of things. Unfortunately, as of today, that evidence just doesn’t exist.

(Small caveat here: if you are severely malnourished and depleted, some of these supplements might help get you back to baseline. But this would only be the case for a small minority of women.)

In a recent market scan, scientists at the Center for Science in the Public Interest dug for evidence of effectiveness for 39 different women’s fertility supplements (you can see the full list here). They scoured company websites and contacted company service reps directly to get their hands on data showing these supplements did anything they claimed — made women ovulate more regularly, shortened TTC, prevented miscarriages, or otherwise helped cure infertility.

To their credit, a (very, very small) minority of companies did actually reference scientific studies. But overwhelmingly to their discredit, not a single referenced study found positive effects on ovulation, time to pregnancy, or pregnancy outcomes in women. In fact, most didn’t even look at these outcomes. One company referenced one published study (yay) that was conducted 78 years ago (yikes)… in men (double yikes).

The majority of companies pointed the investigating scientists to their customer reviews as evidence for their product being effective. While people’s experiences and opinions are certainly valuable (I for one spend entirely too long reading customer reviews before buying anything!), they are simply not a stand-in for science. They are not a stand-in for all the stuff us researchers nerd out over: large-scale, well-controlled, double-blind studies assessing different clinical endpoints like ovulation frequency and regularity, time to pregnancy, or pregnancy outcomes.

The danger in these so-called infertility supplements doesn’t lie in their ineffective chemical composition, but rather in the false sense of hope they may engender for those trying to conceive. Many of these supplements position themselves as “perfect natural alternatives to fertility drugs. ” While FDA-approved fertility drugs might not be “natural,” we know they work and we know how they work — if we didn’t know these two key things, the drugs wouldn’t have gotten the FDA stamp of approval. Further, by suggesting women use infertility supplements rather than science-backed treatments, women are spending time and money on these supplements, both of which are precious and finite resources when it comes to reproductive health.

Infertility supplements: Takeaways

Supplements that claim to do things like cure infertility aren’t up to snuff when it comes to being backed by good research. If you or your partner think you may have a condition that could interfere with fertility, talk to a doctor who can help you get to the bottom of things and direct you toward treatments and interventions that do get the science thumbs-up.

Your best advocate in the fertility supplement world = you

Some companies may make some pretty bold claims about their products — and many claims around fertility often aren’t backed by science. It’s important to do your research on the company and formulation before spending money on something that might not work the way you’re hoping it will.

If you’re planning on trying to get pregnant in the next year or so, you can schedule a preconception appointment and talk to your healthcare provider about any supplements you’re considering. While prenatal vitamins will absolutely be a top recommendation, they might be able to provide insight into any other supplements or vitamins you’re thinking about taking. Make sure to mention all the medications, vitamins, and supplements you’re currently taking so they can keep all that in mind while providing guidance.

Taking a prenatal supplement is one major way to get proactive about your fertility. If you’re looking for more ways to plan ahead, you can consider things like fertility hormone testing to check in with your hormones. We have entered the new millennium. Each such transition is accompanied by formidable predictions, prophecies and yet hopes for the best. But now humanity is at such a point in its development, at which each of us must make a choice: in what world and how will our children be born and live?

Looking back, we see that a person was very dependent on the traditions and spirit of his people, clan, family, he was less free as a person. And humanity in its development swung to the other extreme – individualism and isolation.

Much of what has always been part of family life has been alienated and put on the conveyor belt: food that we don’t grow, and sometimes don’t cook, raising children, whom we entrust to strangers in nurseries and kindergartens, and, probably, the most most importantly, childbirth . This intimate process was also put on stream in public institutions – maternity hospitals. A unique event was sacrificed to not always correct scientific views.

Changes in our century are taking place at a rate that does not correspond to human nature. Modern science forced us to look at our nature from a new, more detached point of view, but it is just beginning to comprehend the subtleties of prenatal 1 and generic processes, although intuitive knowledge about their influence on all human life has always existed, which is confirmed by the ancient epic and mythology.

The atomic age made man lose this intuitive knowledge and find it again as a conscious scientific one.But the price was dear. The victim was three generations of mankind, whose lives, health and destinies were crippled by the technological process of maternity hospitals. The reward is a return to understanding the natural wisdom of natural delivery. Knowledge of the truth makes you free. Now mother, father, child and midwife can enter into co-creation with Nature no longer as slaves of certain scientific views, but as friends.

More than thirty years ago, the movement of spiritual obstetrics was born in Russia. Then it was a few enthusiasts, now – tens of thousands of families across the country.

Therefore, precisely today, when the issue of birth is recognized as one of the most important in the world, we in Russia have the opportunity to choose the path along which we will go in obstetrics.

Moving from old maternity hospital conditions to new ones, it is important not to repeat the mistakes that have already been made and are still being corrected in the West – the flow of instrumental, medication and impersonal childbirth.In order for this not to happen, both existing and new information about the grave consequences of such childbirth and that there is another way – natural and conscious childbirth – is necessary. This is not a return to the old, but a transition to a new level, a new round in the development of mankind. This is the requirement of today.

We believe that the problem of birth is fundamentally related to many problems of today’s world: lack of spirituality, crime, drug addiction, suicide and others. From how a person is born, his future life largely depends. The insanity of war and the aggressiveness of politics have the same roots.

The same connection exists with another major problem of our time – environmental issues.

The pathological desire of modern man to destroy the world around us, our common planet, without thinking about the consequences, reflects the era of clinical obstetrics. When we do not help to establish an intimate connection between mother and child, we do not protect it, we thereby archetypically sever the relationship of humanity with Mother Earth.

A child born in love and joy in a family that has deliberately made its choice will be able to stay in harmony with itself and the world and prevent its destruction.

It is these people who are very much needed today!

Six years have passed since the first edition of our book. Much has changed in our country and in the world.Views changed, children were born. Some of them were fortunate enough to be born at home, thanks in part to this book. Alishani and I, living in different hemispheres, had daughters. Both under the hot sun of Hawaii and under the Moscow sky, mothers continue to give birth to their children as nature and God intended.

However, a person, more and more interfering in various natural processes, did not bypass childbirth. The number of cesarean sections is dramatically increasing in the world.A new generation is born that does not know natural birth. What awaits us as a result of this, no one knows yet. But such interference in human nature cannot pass without a trace.

Fortunately, in Russia, the percentage of caesarean sections is still not so high. But the domestic system of obstetrics has already turned to the wrong path of instrumental, medication and impersonal childbirth, followed by the West, and soon we can come closer to the same picture, when most children are not born, but are surgically removed from the mother.

However, what is happening in our maternity hospitals can hardly be called a natural birth. There is a profanation of the idea of ​​natural childbirth, which they began to call any non-operative childbirth, be it at least three times medication. Doctors think more and more about how they fit a certain pattern when taking delivery, rather than about what happens to each particular woman giving birth. A woman began to be seen as a client who is given freedom for his money within certain limits, mainly in non-fundamental issues.The more we develop medicine as a business, the more it starts earning for us, our births and our children. Thus, women continue the tragic chain of drug delivery, which has been going on for the fourth generation, and our children are born less and less healthy. Before our very eyes, we are losing the opportunity to take a different path in obstetrics – the path of soft, humane, truly natural childbirth.

Like mushrooms after a rain, various centers and courses for preparing for childbirth, organized by people who are very far from the very concept of “natural and conscious childbirth”, began to multiply. Often such centers are organized at maternity hospitals or antenatal clinics. It would be more accurate to call such courses “courses of preparation for the correct behavior of a woman in labor in a maternity hospital” or “courses that teach how not to interfere with the doctor during childbirth.”

But in the midst of commerce and medical routine, there remain those who have carried the ideas of natural birth since their inception and preserved the traditions of humane obstetrics, as well as their followers. They know firsthand about natural childbirth, having their own experience of birth in joy.They have helped and are helping women to become happy mothers.

However, there is still no legal basis in our legislation for the free choice of the place of birth of a child. A woman is forced either to be within the strict framework of the medical system in a maternity hospital or to do without medical assistance during a home birth, if such is suddenly needed. A paradoxical situation is created: you can give birth at home, but you cannot take delivery at home. And midwives who are ready to help parents in home births (and help them, no matter what!) Are outlawed.

Society creates a negative image of home birth as a kind of exotic and dangerous alternative, while forgetting that this is precisely the most traditional type of childbirth – the adoption of a child at home. This type of obstetrics is thousands of years old. And the practice of universal “medical treatment in a public institution” has developed quite recently.

In some Western countries (Holland, Great Britain, etc.), they return to the most humane type of birth – family, home.For a healthy woman and her baby, such childbirth is safer than a maternity hospital. Countries that care about their future generation have long practiced formal home birth. We stubbornly continue to step on the same rake, not noticing the dire consequences.

All this leads to a confrontation between medicine and supporters of soft natural childbirth. But we call for cooperation, not competition. Let’s really leave the “cesarean cesarean”, and the home to home.There is no need to reinvent the wheel. In the WHO recommendations on obstetrics, it is written in black and white: “ Every woman can choose any type of childbirth service that she prefers (any position is possible: standing, kneeling, squatting, in the clinic or at home, in the water or” dry ” ) “. Let’s join those who understand the full significance of the birth process for a MAN.

It is in our power to change the current situation. Look for true information about what happens in drug delivery and its consequences.Demand the fulfillment of your requirements in maternity hospitals. Share positive experiences with gentle birthing. I know a couple who, with a story about their home birth, prompted almost two dozen of their acquaintances to the same happy birth.

Look for family offices if you need to give birth in the hospital. Even in a hospital setting, you can create a gentle birth environment, similar to family clinics in the West. In the future, family maternity centers will surely appear, where there will be a different approach to childbirth than the one that exists now, if a demand for them appears in society.

Do not leave it to doctors or anyone else to decide how to have your baby! And be responsible for your decisions. Remember, your experience can attract or scare away dozens of followers. Not only yours, but also other children are waiting for your decision and your actions.

But many pregnancies are unplanned, and sometimes even a complete surprise.

When a couple wants to have a baby, there are many ways to prepare for a successful conception and healthy pregnancy.

A positive lifestyle, an active lifestyle, clean air, a well-balanced diet, clean water, as well as sunlight, relaxation and communication with nature and beauty in all its manifestations are all important aspects of a healthy lifestyle.The overwhelming environment for you, unpleasant people and, of course, alcoholic beverages, smoking and any kind of drugs harm your health and do not favor a full conception and a healthy pregnancy. Remember that a healthy lifestyle not only contributes to the conception of healthy children, but also increases the chance of conception in general. Since it is very difficult to provide ideal living conditions in most places in Russia today, it becomes especially important to put a healthy lifestyle at the forefront.Perhaps for this you will have to purposefully engage in building your life, strive to interact with people who also highly value these goals, in order to join them in deeds that serve the good of all.

When to start? It’s never too early. It will be a manifestation of wisdom for any potential parents to strive to live in accordance with this image, and not only in the physical plane, but in all respects.

By leading a healthy lifestyle, we thereby enrich the soil on which the seed of new life will grow and develop.This attitude towards the unborn child can broaden our attitude towards the mystery of birth and death. Realizing that birth is not the beginning of life, we can come to the point that death is not the end. When we plant a seed in fertile soil, it will have more strength to rush to the Sun. Love is the light that makes children grow and thrive.

According to the ancient beliefs of the Slavs (which coincides with the ideas of other Indo-European peoples), the body is the essence of the temporary dwelling of the soul, it is contained in it at the birth or conception of a child and leaves at the death of a person.The soul is immortal and is involved in a circle of endless reincarnations.

Outpourings of love on future parents and their environment precede the arrival of a new person in the world. Love and sincere emotional relationships between expectant mothers and fathers are an important basis for the arrival of a new soul in the earthly world. Paying tribute to the role that love plays in conception, we change the way we view intimate relationships in general. When conscious conception becomes the highest manifestation of love, we are ready to join one of the most significant revelations of our time – conscious childbirth .

Difficulties with conception arise for various reasons. From a medical point of view, the impossibility or difficulty of conception can be due to various reasons:

  • complete absence or insufficient content of sperm in the sperm of a man, as well as their lethargy;
  • problems with ovulation or lack of ovulation in a woman, obstruction of the fallopian (fallopian) tubes;
  • hormonal incompatibility of a particular pair of healthy people,

and many others.

In fact, the roots of the problems lie not in the physical, but in much more subtle areas. Your relationship in a couple, relationships with relatives, the true reasons for the desire to have a child, sometimes hidden even from ourselves, the inner attitude, the unwillingness to accept the child not as a thing, but as a gift from God, as an incarnating Soul, can be reasons that prevent conception. Our physical body only manifests them in one way or another. Start by working on yourself and your inner world. By changing spiritually, you will open the doors for the arrival of the unborn baby.

Support your inner changes on the physical plane. We can give some advice here.

1. The spouse can go to the doctor to examine the quality of the sperm, that is, to determine the number of full-fledged motile sperm. Insufficient sperm count or decreased sperm activity may be the result of functional impairment, for example, due to varicose veins of the scrotum or increased temperature (overheating) of the scrotum.Sperm cannot be produced when the temperature is too high. Overheating from excessively hot showers, baths, baths, underpants or trousers that are too tight can interfere with sperm production.

Encourage your husband to take cool showers several times a day, especially shortly before sexual intercourse. Dousing with cold water will also help. It is well known that foods rich in vitamins and micronutrients (for example, herbs, nuts, fresh vegetables and fruits, seafood and some medicinal herbs) significantly improve the quality and quantity of sperm.

Factors that worsen sperm quality can be stress, lack of rest, drinking and smoking, taking certain medications, etc.

2. If a woman’s menstrual cycles are irregular, then ovulation is also impaired. Conception can only occur during ovulation, which usually occurs 14 days before the start of the next period.
