Salicylic acid is a medication that helps the skin peel. It may also remove psoriasis scales and help the patches of skin heal faster.
Stronger treatments contain more salicylic acid. However, this might irritate the skin. People may wish to try starting with a product that has relatively low salicylic acid content and gradually increasing the strength based on how their skin reacts to it.
Shampoos containing salicylic acid also support other scalp psoriasis treatments, such as topical corticosteroids.
Some other acids, including glycolic acid and lactic acid, can also help the skin peel. However, much like salicylic acid, they can irritate the skin at higher concentrations.
Ketoconazole
Ketoconazole is an antifungal agent that can also help people with the dry, scaly skin associated with psoriasis. Shampoos that contain ketoconazole can help remove both dandruff and psoriatic plaques.
Some people with scalp psoriasis have a higher risk of fungal infections than people who do not have the condition. Ketoconazole is an especially beneficial ingredient that can protect the scalp from painful yeast infections.
Selenium sulfide
This is a preparation that people use to treat many conditions that affect the skin, including hyperkeratosis and seborrheic dermatitis.
Research has indicated that selenium sulfide is also effective for treating scalp psoriasis.
Tar
Some people use coal tar to treat psoriasis, including scalp psoriasis. Although researchers have carried out more studies into coal tar and psoriasis, pine tar soaps and shampoos may also work.
It is also possible to wash the scalp with tar soap. However, the soap has a strong, distinct smell and can be irritating to the skin. One 2016 review concluded that the evidence for using coal tar is not yet strong enough to confirm its benefits.
People should start with a low concentration and work up to more potent formulas. However, it is important for people to check with a doctor about the maximum strength they should use.
If shampoos and milder treatments are not effective, a doctor might consider prescribing a more intensive topical treatment, including:
- calcipotriene (Dovonex), which is a synthetic form of vitamin D-3 that helps by slowing cell growth in the skin, shedding scales, and reducing the size of lesions
- calcipotriene and betamethasone dipropionate (Taclonex), which can help make lesions flatter, reduce skin cell production, get rid of scales, and soothe itching
- anthralin (Zithranol-RR), which helps slow skin cell production
- tazarotene (Tazorac), which is a topical retinoid that helps reduce the speed of skin cell growth
A doctor might also consider systemic therapies. These are medications that affect many systems in the body and target the autoimmune activity at the root of psoriasis.
However, they will only recommend these in very severe cases of scalp psoriasis that do not respond to other treatments.
Treatment might become less effective if a person uses the same medication for a long period of time. A doctor may need to change their treatment regimen if this is the case. People can rotate a combination of treatments for the best long-term results.
No home remedy can resolve scalp psoriasis on its own. However, certain natural and alternative treatments can help improve the appearance of psoriasis lesions, moisturize the skin, and soothe itching when a person uses them alongside more traditional treatments.
The National Psoriasis Foundation recommend the following herbal remedies for psoriasis:
Turmeric
Some research in rats has suggested that turmeric can reduce the inflammation associated with psoriasis.
A 2018 randomized controlled trial suggested that turmeric tonic has anti-inflammatory effects in those who have scalp psoriasis. The study authors reported satisfactory results supporting its use as a treatment.
Apple cider vinegar
Apple cider vinegar can help relieve itching. People can buy a bottle from the store and apply it to the scalp a few times per week.
If apple cider vinegar causes a burning sensation, rinse it after it dries or dilute it 1:1 with water. People whose skin is cracked and bleeding should not try this remedy.
Dead Sea salts or Epsom salts
People can try mixing either of these with warm bathwater and soaking in the solution for around 15 minutes.
The salts might help a person remove scales and relieve itching. People who try this remedy should remember to moisturize the area afterward.
Aloe vera
Aloe vera is a plant-sourced gel that can help soothe red skin and reduce scaling in people with psoriasis.
Avoid taking oral aloe supplements, however, as they provide no benefit and may carry health risks.
Oats or colloidal oatmeal
Oats can soothe irritated skin and may help with extremely dry plaques on the scalp.
For most people, an environmental factor triggers a flare in the immune system, causing patches of psoriatic plaque to develop.
Triggers vary between people, but common triggers include:
Some people also notice that their psoriasis symptoms get worse after eating certain foods or as a result of allergies. Research has not yet conclusively proven that these factors can trigger psoriasis, however.
It is worth noting that environmental triggers alone do not cause psoriasis. People without a genetic predisposition for psoriasis will not develop it, even when they experience these triggers.

Psoriasis is a group of autoimmune conditions that affect the skin. Autoimmune conditions cause the immune system to target the wrong cells or overreact to harmless triggers.
In the case of psoriasis, the autoimmune activity causes skin cells to grow more quickly than usual, creating scales and lesions on the skin. The appearance of the lesions depends on the type of psoriasis.
The only type of psoriasis that affects the scalp, however, is plaque psoriasis.
The most common symptoms of scalp psoriasis include:

Without treatment, psoriasis can lead to serious complications, such as heart disease and certain types of cancer, including head and neck cancers and malignant growths in the digestive tract.
Living with psoriasis can affect a person’s self-esteem and may give rise to depression or anxiety. For this reason, people with psoriasis may wish to try out some positive coping strategies to boost their well-being.

3 ingredients to Look For
The following ingredients can help reduce itchiness and flakes, as well as gently soften and remove thick scale.
- Hydrocortisone is a mild corticosteroid that can decrease itching and inflammation.
- Coal tar decreases skin cell growth and helps the scalp shed dead skin cells.
- Salicylic acid is a “keratolytic,” or peeling agent, that helps soften and remove thick scale. (Urea and lactic acid work similarly.) Once scale is removed, other medications will be able to penetrate the skin more easily, potentially increasing effectiveness.
How To Tell The Difference
If your scalp itches and flakes, your doctor can tell if it’s just dandruff or a more serious problem like psoriasis, a disease that causes red, scaly patches on your skin. Once you have the right diagnosis, you can treat the cause and get some relief.
What Is Dandruff?
Dandruff is a common skin problem. You may notice flakes that fall off your scalp and cling to your hair or land on your clothing. Your scalp may itch, too.
Several things can cause dandruff:
- Seborrheic dermatitis: This is oily, itchy, irritated skin that flakes off on your scalp. It also can happen with your eyebrows, groin, or chest hair.
- Contact dermatitis: Hair care products like shampoo, gel, or dye can irritate your scalp and cause redness, itchiness, and flakes.
- Fungus called malassezia is a yeast that thrives on the oil on your scalp.
- If you don’t shampoo your hair often enough, oily skin can flake off.
- Dry skin can lead to small flakes on your scalp. You’ll probably have dry skin all over your body.
- Male hormones:Men are more likely to get dandruff than women.
- People whose immune systems can’t fight off diseases, for instance people who have HIV, may be more likely to get dandruff.
Dandruff usually isn’t serious. You can’t catch it from anyone else or pass it on. It can be uncomfortable or embarrassing, though. Dandruff can be treated at home without a prescription.
What Is Scalp Psoriasis?
Psoriasis can affect your scalp, and the red, scaly patches it causes can flake off like dandruff does. There are a few differences, though:
- It’s chronic: Psoriasis is long-lasting while dandruff may come and go.
- It’s more scaly than flaky. If it’s mild, scalp psoriasis looks like scaly, silvery, or powdery patches that may come off in tiny pieces. More serious outbreaks can be red and painful.
- It may spread. Psoriasis patches can creep past your hairline to your forehead, the back of your neck, or the skin around your ears. You may have psoriasis patches on other parts of your body, too, like your elbows, legs, feet, palms, or back.
- It’s an autoimmune disease. Psoriasis is caused by your body’s immune system: White blood cells that should fight off diseases attack your skin cells instead.

Diagnosis
Your doctor may figure out the reason your scalp flakes or itches just from your symptoms. To be sure, they may look at a small piece of skin from your scalp under a microscope or send it to a lab.
Treatment
If you have mild dandruff because your scalp is greasy or oily, you may just try a regular, gentle shampoo.
If that doesn’t help, some shampoos are made to control dandruff. They may have zinc pyrithione (Head & Shoulders, Free & Clear), coal tar (Neutrogena T/Gel), salicylic acid (Neutrogena T/Sal), selenium sulfide (Selsun Blue), or ketoconazole (Nizoral). Tea tree oil is an alternative treatment for dandruff. Follow the directions on the shampoo bottle.
Your doctor or pharmacist can point you to the right shampoo for you. You also can get a prescription dandruff product if over-the-counter shampoos don’t stop your itch and flakes.
Coal tar and salicylic acid shampoos or scalp treatments may also help with mild scalp psoriasis.
Topical creams, ointments, and foams like the following can slow psoriasis skin buildup and ease red, scaly patches on your scalp. They may have vitamins or steroids to calm the inflammation:
- Anthralin (Zithranol-RR)
- Calcipotriene (Dovonex)
- Calcipotriene and betamethasone dipropionate (Taclonex)
- Calcitriol (Vectical)
- Tazarotene (Tazorac)

Your doctor can also put steroids (strong anti-inflammatory drugs) into the patches on your scalp if your psoriasis is milder or just in a few spots. If you have severe psoriasis, you may need stronger drugs. These include methotrexate, which affects how certain cells grow; cyclosporine, which slows down your immune system; biologics, which target specific areas of your immune system; or oral retinoids, which are high doses of vitamin A.
You can also try ultraviolet or UV light treatments to control your psoriasis patches. You can part your hair in rows so UV light from a special lamp can reach your scalp or use a handheld UV comb that beams the light directly to your scalp.

What is scalp psoriasis?
Psoriasis is a common skin condition, characterised by red scaly thickened patches (plaques). It often affects the scalp. Scalp psoriasis may occur in isolation or with any other form of psoriasis.
The back of the head is a common site for psoriasis, but multiple discrete areas of the scalp or the whole scalp may be affected. Scalp psoriasis is characterised by thick scale over well-defined, red thickened skin. The scale is often silvery white. Psoriasis may extend slightly beyond the hairline (facial psoriasis).
Scalp psoriasis, even though often adequately camouflaged by the hair, is often a source of social embarrassment due to flaking of the scale and severe ‘dandruff’. Scalp psoriasis may not cause any symptoms at all or may be extremely itchy. It tends to be a chronic problem, lasting many years, although it often fluctuates in severity and extent.
In very severe cases, there may be some temporary mild localised hair loss, but scalp psoriasis does not cause permanent balding.

Sebopsoriasis
Sebopsoriasis is an overlap between psoriasis and another common skin condition, seborrhoeic dermatitis. Sebopsoriasis tends to have less silvery scale than psoriasis and more yellowish, greasy scale.
Pityriasis amiantacea
Pityriasis amiantacea is characterised by thick, yellow-white scales densely coating the scalp skin and adhering to the hairs as they exit the scalp. The scales are arranged in an overlapping manner like tiles on a roof or flakes of asbestos, hence the name. The underlying scalp skin may appear normal, aside from the scale, or can be reddened or scaly. Pityriasis amiantacea is often present without any apparent underlying cause but may be associated with psoriasis, seborrhoeic dermatitis or lichen simplex (another form of dermatitis).
Psoriatic hair loss
Most patients with scalp psoriasis do not lose hair despite thick plaques. However, hair loss and localised bald patches (alopecia) can occur.
- Psoriatic alopecia often affects psoriatic plaques (ie, red, scaly, thickened skin)
- Scratching, combing, pulling off scale can contribute by pulling out clumps of hair
- The hair usually completely regrows after some time
- Scarring alopecia due to psoriasis is rare
- Generalised hair shedding can occur (telogen effluvium)
- Psoriasis may be associated with other autoimmune diseases including alopecia areata
- Treatment might lead to hair loss, for example, due to the oral retinoid acitretin or biologics

Scalp care
Scalp psoriasis requires slightly different regimes from psoriasis affecting the skin elsewhere. This is due to hair, which makes the application of many topical products difficult and protects the scalp from the effects of ultraviolet light. Unfortunately, many scalp treatments for scalp psoriasis are messy and smelly. Most treatments will need to be used regularly for several weeks before a benefit is seen.
Special medicated shampoos can be purchased from the chemist.
- Coal tar shampoos are suitable for most patients with scalp psoriasis.
- Ketoconazole, ciclopirox, zinc pyrithione and other antifungal shampoos are effective for dandruff and seborrhoeic dermatitis. They have a varying effect on sebopsoriasis and psoriasis.
The shampoos work best if rubbed into the scalp well, and left in for 5 or 10 minutes and then reapplied. They are safe for daily use but may irritate if applied more than twice weekly. If you dislike the smell of coal tar, try shampooing again with a favourite brand, and use a conditioner.
More severe cases require leave-on scalp applications.
- Alcohol-based, foam, gel or lotion forms of topical steroid and calcipotriol can reduce redness and itch, but they don’t lift scale very well. Use topical steroids intermittently; overuse results in more extensive and severe psoriasis.
- Salicylic acid and coal tar creams work much better but are messy. Coconut oil compound ointment is a combination of coal tar, salicylic acid and sulphur and seems particularly effective. Leave on for at least an hour and shampoo off later. Most people rub the cream into the plaques at night and wash it off in the morning.
- Dithranol may be effective but is difficult to use and may be messy as it stains hair and fabrics.
Use the scalp preparation daily at first then as the condition improves, reduce the frequency. In many cases the scale soon builds up again, so the creams may have to be applied regularly to keep the scalp clear. Topical steroids are best used only 2-3 times weekly, long term to avoid complications.
Cutting hair short helps control scalp psoriasis, probably by making the treatments easier to apply, but is not appealing to everyone.
Phototherapy is effective for chronic plaque psoriasis but challenging to deliver to the scalp. Special targeted devices and UVB combs have been devised, and appear very helpful. In some cases, prolonged clearance has resulted from a course of treatment.
Intralesional corticosteroid injections and systemic therapy may be justified for patients with severe scalp psoriasis that has failed to respond to topical treatments and targeted phototherapy. These include acitretin, methotrexate, ciclosporin and biologic agents.

Scalp psoriasis can be challenging to treat, but there are ways to alleviate the itching, scales, and irritation that tend to go hand-in-hand with the condition. First up: Make sure you’re using the right shampoo. To learn more about the best shampoos for scalp psoriasis, Bustle spoke with board-certified dermatologist Dr. Tess Mauricio, M.D., who recommends rotating shampoos that contain coal tar, salicylic acid, ketoconazole, and zinc pyrithione, which are some of the most effective ingredients for treating scalp psoriasis out there.

Also, since stress is highly correlated with scalp psoriasis flareups, Dr. Mauricio says that managing stress is very important — so, though easier said than done, anything you can do to reduce your stress levels is great. Finally, if over-the-counter shampoos aren’t working, consult with your dermatologist. They can prescribe topical corticosteroids and discuss other treatment options that may be more effective.

“Coal tar is a mainstay in the treatment of psoriasis,” says Dr. Mauricio, adding that coal tar shampoos, such as Neutrogena T/Gel Therapeutic Shampoo, can be very effective. 3. Best Selenium Sulfide Shampoo
Selsun Blue Shampoo, which actually has a dark green color, is another doctor-approved pick for treating scalp psoriasis. It contains 1% selenium sulfide, a medication that works to treat a variety common scalp and skin conditions, including seborrheic dermatitis. This also has a nice, cooling feel, thanks to the menthol in the formula.
4. Best Salicylic Acid Shampoo
